If you need help getting to your doctor’s offi ce, you can receive FREE
help getting there by calling the Medicaid Transportation
Brokerage before your doctor’s appointment. They are open Monday-Friday from 8AM to 5PM.
You can call them at 1-800-752-9422.
When you call, have these things handy:
- Your DSHS Medical ID Card (coupon)
- Your date of birth
- Your address
- Name, address and phone number of the clinic
- Exact appointment date(s) and time(s)
These services do not apply to Basic Health Subsidized.
