Do you need a ride to your healthcare appointment?

If you need help getting to your doctor’s office, you can receive FREE help getting there by calling the Medicaid Transportation Brokerage before your doctor’s appointment. They are open Monday-Friday from 8AM to 5PM. You can call them at 1-800-752-9422.

When you call, have these things handy:

• Your Provider One Services Card
• Your date of birth
• Your address
• Name, address and phone number of the clinic
• Exact appointment date(s) and time(s)

These services do not apply to Basic Health Subsidized.
