Living Well with Chronic Conditions
Living Well with Chronic Conditions is the chronic disease self-management
workshop designed and evaluated by Stanford University. It is a six-week workshop
for people with chronic disease and their family or friends. Classes are taught
by two trained leaders, at least one of whom has a chronic condition. Evaluation
has shown that the workshop helps participants improve their health status and reduce
health care utilization.
Classes are highly participative. Mutual support and success build confidence in
their ability to manage their health and maintain active, fulfilling lives. Subjects
covered in the workshop include techniques to deal with frustration, fatigue, pain
and isolation; appropriate exercise for maintaining and improving strength, flexibility,
and endurance; appropriate use of medications; communicating effectively with family,
friends, and health professionals; nutrition; and evaluating new treatments.

CUP can work with your organization to ensure successful implementation of chronic
disease self-management resources in the communities you serve. We offer:
· Four-day training for new self-management workshop leaders
· Leaders can hold workshops in your facility
· Ongoing technical support regarding licensing, recruitment, community linkages
and referral systems
· Periodic statewide networking opportunities for leaders
Who is eligible for leader training?
Anyone with a commitment to helping people with chronic health conditions by leading
self-management workshops is eligible. Priority for leader training programs is
given to organizations that:
- Commit organizational resources to support the program (funding for licensing fee,
staff time for training, leading workshops, and developing referral systems);
- Commit to offer at least two 6-week workshops per year; and
- Reach diverse audiences, including low income and high-risk populations.
