Welcome to CUP's Credentialing Department. CUP follows NCQA guidelines to credential
and recredential network practitioners and facilities. CUP accepts any of three
credentialing applications, including those from SW Washington Medical Center, Washington State and
Oregon. Initial credential applications should be mailed to: Michele
Luffman, Credentialing Lead, Columbia United Providers, 19120 SE 34th St.
#201, Vancouver, WA 98683.
CUP is now offering online recredentialing services to our network providers in
an effort to simplify the recredentialing process. To enroll in CUP’s online recredentialing
process, simply contact CUP’s Credentialing Coordinator,
Michele Luffman
at (360)891-1520.
