NO REFERRAL IS REQUIRED FOR HEALTHY OPTIONS AND BASIC HEALTH PLUS PROGRAMS

1. Standard Nebulizer-one unit every other year
2. Nebulizer supplies-tubing, filters, medication/administration cups
3. Peak Flow Meter-Personal Best low/high range or equivalent. May have 2 meters per year
4. Spacers: (with and without masks): Aerochamber or equivalent

Diabetes Diagnosis
NO REFERRAL IS REQUIRED FOR HEALTHY OPTIONS AND BASIC HEALTH PLUS PROGRAMS

1. Diabetic Monitors: True Track Blood Glucose Monitoring System, TrueResult, and True2Go.
2. Supplies: syringes/needles/diabetic/keto test strips, lancets, sharps containers
