Formulary
CHPW-CUP Network maintains a Formulary according to the requirements mandated by the Health Care Authority. The Formulary includes coverage for all therapeutic classes.
As of 7.1.12 CUP is adopting the CHPW formulary. Our Medical Directors and Pharmacists have worked hard to make this an easy transition where differences occur. Should you have any questions or concerns, please contact our Pharmacy Benefits Coordinator at 360.449.8931
