Here is what you need to do:
- Call CUP's Member Services department at (360) 891-1520 and ask for our special sports physical form - or click here: Sports Physical Form
- Call your provider and schedule a Well Child visit for your teen.
- Bring your special sports physical exam form with you to the Well Child visit.
